EOB or Claims questions
If you need assistance regarding any information contained on an electronic copy of an EOB or a specific claim, please call (866) 927-2200 or (563) 327-2200 and enter the extension number of the claims representative listed on the EOB. Please have the EOB number, claim number and any other important information available when calling.

Secure Electronic Correspondence
Sending a standard email that contains personal information or protected health information (PHI) is not a secure means of sending correspondence. Since standard emails are transported via the Internet in a plain text format, an unscrupulous person could extract the information and use it to do any number of illegal acts. Butler has created this tool for you and your employer to send secure correspondence to us, fully SSL encrypted and within the guidelines of the Health Insurance Portability and Accountability Act (HIPAA).
